The exact cause remains unclear. Amongst others, intermittent intraosseous ischemia due to embolic vascular occlusions, infections, traumata, vascuar spasms, vegetative dysregulation, or vascular arrosions are discussed. Lifestyle habits and nutrition seem to play a role, too. There is a high familial incidence.
